*A contact list of Executive Director, Board President, and others persons involved.

2. Has your organization or group received any monetary funds in the past year, either through grants or donations?

3. Are you willing to provide M&D with a database of clients, patrons or persons, (that aren’t confidential) so that we can sell advertising, send postcards, or e-mail them to promote your show? (Remember the more of your people who know the more they will come and the more money you make)
